At Ashore Wellness, we provide comprehensive assessment reports for legal and court-related matters. Our team consists of AASW Accredited Social Workers and ACA Registered Counsellors, who conduct professional assessments and prepare reports detailing an individual’s mental health status, capacity, recovery progress, and available support systems. These reports are tailored to specific legal requirements and can be crucial in legal proceedings.
Important Considerations
Clients must confirm with their lawyer whether a report from an Accredited Social Worker or ACA Counsellor is suitable for their specific legal matter.
We do not provide Functional Capacity Assessments for NDIS purposes at this stage.
Common Report Types
Alcohol and Other Drugs (AoD) Reports
- Treatment Report Letter – A concise summary of an individual’s engagement in treatment, progress, and recommendations for future care.
- Comprehensive Treatment Report – A detailed report covering the individual’s substance use history, treatment adherence, recovery efforts, and ongoing support needs.
Domestic Violence (DV) Reports
A thorough assessment including:
- Incident history and evaluation
- The psychological impact of domestic violence trauma
- Recovery progress and available support systems
These reports may also be used to support visa applications in cases where domestic violence is a contributing factor.
Family Court Reports
Anassessment that supports individuals involved in family law and child protection matters, providing:
- An overview of their current situation
- An evaluation of parenting capacity, including strengths and potential risks in their caregiving environment
- Consideration of support systems and interventions in place
These reports can be used in family law disputes and custody proceedings.
Functional Capacity Assessment (FCA) for NDIS purposes
A Functional Capacity Assessment (FCA) can be used as an evidence for your NDIS application or review. Our social workers are eligibile to conduct FCA for
- Primary Disability – Psychosocial
- Primary Disability – Other

Other Reports
We also provide other type of reports such as psychosocial assessment reports to support individuals involved in various court-ordered matters, report letter relating to Revocation of Self-Exclusion, or other types of reports.
Process & Requirements
- For existing clients: If you are already accessing support at Ashore Wellness for AoD addiction recovery or other relevant concerns, our team can prepare reports based on progress made during sessions.
- For new clients: If you have not yet received support from Ashore Wellness for the specific issue requiring a report, you will need to attend at least 3+ hours of consultation sessions. This allows our team to thoroughly assess your case before preparing the required report.
Fees
Below are just an estimate accurate in July 2025. Actual fees may vary depending on your situation. Please get in touch with us for more details.
- Typical Report Letter (1 to 2 pages): $450
- Comprehensive Report (8 to 20+ pages): $1200 – $2500+
Please note that report writing fees are separate from consultation, interview, and counselling session fees. If you require Affidavit in addition to the report, additional costs will occur for time spent on both making the affidavit and the process in getting it certified.

Frequently Asked Questions
How long does it take to complete an assessment report?
The timeline depends on the complexity of the case. A Treatment Report Letter may take 3-5 business days, while a Comprehensive Report may take 2-3 weeks. Urgent requests may be accommodated at an additional cost.
Do I need a lawyer to request a report?
While you do not need a lawyer to request a report, it is highly recommended that you check with your legal representative to ensure that a report from an Accredited Social Worker or ACA Counsellor meets the requirements of your case.
Can I request a report if I am not a current client of Ashore Wellness?
Yes, but new clients will need to complete at least 3+ hours of consultation sessions so that we can thoroughly assess the situation before preparing the report.
Are your reports accepted by all courts and legal authorities?
Acceptance depends on the specific legal requirements of the court or agency handling your case. Reports from Accredited Social Workers are generally accepted in various legal contexts, and some accept reports from ACA Counsellor too.
It is advisable to check with your lawyer.
Can I request an urgent report?
Yes, urgent reports may be accommodated depending on availability. Additional fees may apply for expedited requests.
Do you provide Functional Capacity Assessments for NDIS applications?
Yes, our social worker can conduct Functional Capacity Assessment (FCA), as evidence to present for your NDIS application.
NOTE: We can only conduct FCA if the primary disability type is either Psychosocial Disability, or Other Disability. Can I make changes to the report once it is completed?
Reports are based on professional assessments and factual information. However, if you believe there is an error, you may request a review. Any modifications will be made in line with professional and ethical guidelines.
